To support practices with identifying Hypertension patients by 'at-risk' groups, the following Ardens searches are available. Further NHS England guidance can be found here. It is recommended that all clinical commissioning groups (CCGs) support primary care networks (PCNs) to implement blood pressure (BP) monitoring for patients with a diagnosis of Hypertension, to allow treatment to be optimised where necessary.
